Welcome to Open Enrollment 2020!

We want to help make Open Enrollment to be convenient and easy for you.

Follow the step-by-step enrollment guide below. Each step follows along with your Open Enrollment Booklet that you received in the mail along with your Open Enrollment letter.

By following each step, you will quickly navigate through each of your benefit options and be able to decide if you need to make any changes.

You can make your selections securely, quickly and easily online by logging into eBenefits on the City of San Francisco's Employee Portal or you can return an Open Enrollment Application Form.

Open Enrollment ends on October 30, 2020 at 5pm PST.


City and County of San Francisco employees can make all their Open Enrollment elections online using eBenefits.

After you review your plan documents, make your selections securely, quickly and easily online 24/7 through the month of October by logging into eBenefits from the City's Employee Portal.

Step 1 - Review your Open Enrollment Letter and Rates

Review your Open Enrollment Letter for current medical, dental and vision elections and new 2021 rates.

If you have changes you would like to make, proceed through the steps below.

If you do not have any changes you would like to make, please go to Step 2 below if you would like to enroll in a Healthcare or Child Care Dependent Care FSA and Step 3 to see if you need to add or drop dependents.

Otherwise, no further action is required, and your current elections will roll over into 2021 (except for FSAs which expire on December 31, 2020 and you must re-enroll in every year).

Step 2 - Flexible Spending Accounts (FSA)

If you currently have an FSA, your election will not extend into next year. All current FSA elections will expire on December 31, 2020. If you would like an FSA in 2021, you must re-enroll in a new FSA during the current Open Enrollment period.

2020 Healthcare FSA balances between $10 and $550 will automatically carryover and be available for expenses in 2021. This does not apply to Child Care Dependent Care FSAs. 

If you would like to set aside pre-tax dollars for upcoming Healthcare or Child Care Dependent Care expenses, determine how much you would like to set aside and complete the Choose a Flexible Spending Account page in eBenefits.

To login to your FSA account online or if you have any questions about FSAs, visit our FSA administrator, P&A Group at or call (800) 688-2611.

Step 3 - Add or Drop Dependents

Review the dependent(s) listed in your enclosed Open Enrollment letter. If you need to add or drop a dependent, review the dependent eligibility rules here.

Complete the Review Dependents page in eBenefits to add dependents or modify existing dependents. You will need to submit copies of supporting documents.

Remember, new dependents must have supporting documentation submitted with their elections in order to be enrolled (e.g. birth certificate, certified marriage certificate).

What you should know when adding dependents

When adding dependents, remember that not all relationship types are eligible to receive benefits. You can list other relationships that are not eligible for benefits as emergency contacts. 

To find out if your dependent is eligible for coverage, please refer to the Eligibility Relationship Types table.

Step 4 - Voluntary Benefits

If you would like to enroll in voluntary benefits, refer to page 9 in your Open Enrollment Booklet to review the different voluntary benefits available.

To enroll in voluntary benefits, you must contact WORKTERRA at (866) 528-5360 or visit to self-enroll, disenroll, or confirm any existing elections.

Step 5 - Making Changes to Your Health Plan Benefits

Start by reviewing the Service Areas of the medical plans available to you here. Review coverage details on pages 4 and 5 of your Open Enrollment Benefits Booklet and review the rates for available plans in your area on page 2 of your Open Enrollment letter.

Select your plan and complete Choose a Medical Plan page in eBenefits.

Medical Plans

Active members can select from the following medical plans: UnitedHealthcare PPO (City Plan), Blue Shield of California's Trio HMO or Access+ HMO and Kaiser Permanente HMO.

Blue Shield of California HMO

Step 6 - Vision Benefits

Review the Vision benefits options and rates on page 6 of your Open Enrollment Booklet. You must be enrolled in a medical plan to receive basic vision benefits.

Enrollment in the VSP Premier Plan requires that all dependents enrolled in a medical coverage be enrolled in the VSP Premier Plan.

Complete the Enroll in a Vision Premier Plan page in eBenefits.

Vision Plans

Active members who enroll in a medical plan are also enrolled into the VSP Basic Plan and included with your medical premium. VSP Premier, which offers enhanced benefits, is available with a premium.

Step 7 - Dental Benefits

Selecting your dental benefits is easy. Review your Dental benefit options and costs on page 7. Complete the Enroll in a Dental Plan page in eBenefits.

Dental Plans

Active members can select from the following dental plans: Delta Dental PPO, DeltaCare USA DHMO or UHC Dental DHMO.

Step 8 - Complete and Submit Your Elections Online

Complete and submit your eBenefits elections online. You can also fax or mail your completed Open Enrollment Application Form and documentation (do not send originals) to SFHSS.

Step 9 - Confirmation Letter

After Open Enrollment ends, you’ll receive a Confirmation Statement in the mail from SFHSS in December. Please review your Confirmation Statement to make sure your benefit elections are correct. Changes made during Open Enrollment take effect January 1, 2021.
