Delta Dental PPO

The Delta Dental PPO plan gives you the freedom to visit any in-network or out-of-network dentist. Delta Dental PPO provides three options for selecting a dental provider. Each option provides coverage for the same types of services, but at different cost levels. The plan pays higher benefits (and you pay less) when you visit an in-network Delta Dental PPO dentist. Please refer to the Evidence of Coverage for a detailed list of covered services, exclusions and limitations.

Delta Dental PPO – Active

After the Benefit Maximum has been reached, services are still available to members at Delta’s Contracted Rate.

Coverage for Dental Implants

Child and Adult Orthodontia
Lifetime Maximum of $2,500 when services are provided by a Delta Dental PPO Dentist.
Lifetime Maximum of $2,000 when services are provided by a Delta Dental Dentist.
Lifetime Maximum of $1,500 when services are provided by a non-Delta Dental Dentist.

Dental Accident Benefits
$10,000 limit

Coverage for Non-IV Sedation

Diagnostic and Preventive Benefits
Diagnostic and preventive benefits do not count toward the annual plan Maximum.

While Pregnant
One additional oral examination and either one additional routine cleaning or one additional periodontal scaling and root planing per quadrant.

Delta Dental PPO – Retiree

After the Benefit Maximum has been reached, services are still available to members at Delta’s Contracted Rate.

Deductibles Waived for Diagnostic and Preventive Benefits

No Deductible when a Delta Dental PPO Dentist is used

Diagnostic and Preventive Benefits
Diagnostic and preventive benefits do not count toward the annual plan Maximum.

While Pregnant
One additional oral exam and either one additional routine cleaning, one additional periodontal scaling and root planing per quadrant, or one additional periodontal maintenance procedure.

Coverage for Dental Implants
Coverage does not include interim implants, endodontic endosseous implants, or extraoral implants.

Coverage for Non-IV Sedation

Periodontal Scaling and Root Planing
Covered at 100%, once every Benefit Year per quadrant, with no more than two quadrants covered on the same date of service.

Routine Cleaning, Scaling in the Presence of Moderate or Severe Gingival Inflammation, and Periodontal Maintenance
Covered at 100%, with any combination of four services per Benefit Year.

SmileWay Wellness Benefits
SmileWay Wellness Benefits are not subject to Benefit Year Maximums.

SFHSS Has Expanded Dental Coverage for Qualified Retirees with SmileWay

SFHSS retirees with a qualifying chronic health condition can opt-in to receive expanded dental coverage with complimentary SmileWay® Wellness Benefits.

This benefit offers 100% coverage for four cleanings, gum maintenance procedures, scaling procedures, or a combination of these three annually. Also included, is 100% coverage for one periodontal scaling and root planing procedure per quarter.

Qualifying conditions include:

  • Amyotrophic Lateral Sclerosis (ALS)
  • Cancer
  • Chronic Kidney Disease
  • Diabetes
  • Heart Disease
  • HIV/AIDS
  • Huntington Disease
  • Joint Replacement
  • Lupus

To enroll in the SmileWay program, call (888) 335-8827 to speak to a representative. The Customer Service hours are Monday through Friday, 8:00 a.m. to 8:00 p.m., PST.

You can also enroll in the SmileWay program online by going to deltadentalins.com/ccsf and logging into your Online Services Account (or create one by clicking on "Register"). Click the Optional Benefits tab in the left column. Click on the Opt In next to the name of the person you want to enroll. You can enroll yourself or a dependent child. Dependents over age 18 must self-elect through their own Online Services Account.
