Group Life Insurance

This form allows you to choose who will receive your life insurance benefits in the event of your passing.

This form allows you to continue your employer-sponsored group life insurance (both basic and voluntary/supplemental) if you leave employment, retire, or otherwise lose eligibility, by paying premiums directly.

This document provides an overview of employer-paid life insurance coverage based on a member’s labor union classification, which is organized into different “Classes" for short.

• Class 1 - $150,000 Life Insurance Policy:
  • Municipal Executives Association (MEA);
  • Elected Officials;
  • Librarian or Assistant Law Librarian;
  • Members of the San Francisco Board of Supervisors;
  • Individual employment contract for the Municipal Transit Agency; or
  • Unrepresented Contract Rte FBP (union code 990
     
• Class 2 - $150,000 Life Insurance Policy:
  • Municipal Attorneys Association (MAA)
     
• Class 3 - $50,000 Life Insurance Policy:
  • Unrepresented Managers or Unrepresented Employees of the City or County of San Francisco;
  • Service Employees International Union (SEIU) Local 1021 and SEIU Local 1021 Staff Nurses;
  • Teamsters Local 856 Multi-Unit;
  • Transport Workers Union (TWU) Local 200 SEAM;
  • International Federation of Professional and Technical Engineers (IFPTE) Local 21;
  • Union of American Physicians and Dentists (UAPD);
  • Machinists Union Local 1414;
  • Members of a union under the Consolidated Crafts Coalition collective bargaining agreement;
  • Deputy Probation Officers Association;
  • United Association of Journeymen and Apprentices of the Plumbing and Pipe Fitting Industry, Local 38;

• Class 5 - $150,000 Life Insurance Policy:
  • Superior Court of California, County of San Francisco Municipal Executives Association (MEA);
  • Superior Court of California, County of San Francisco Commissioners Association; or
  • Superior Court of California, County of San Francisco Unrepresented Managers. 
     
• Class 6 - $50,000 Life Insurance Policy:
  • Superior Court of California, County of San Francisco International Federation of Professional and Technical Engineers (IFPTE) Local 21;
  • Superior Court of California, County of San Francisco Court Reporters; or
  • Superior Court of California, County of San Francisco Unrepresented Professionals or Managers.
     
• Class 7 - $50,000 Life Insurance Policy:
