How to Find the Blue Shield Primary Care Physician ID Number
Those who are not currently enrolled in any Blue Shield plan and are enrolling into Blue Shield Blue Shield Trio HMO or Blue Shield Access+ HMO may elect a PCP. You will need to provide the ID number of your Primary Care Physician (PCP). If you choose not to elect a PCP, Blue Shield will find one for you.
If the provider is unavailable (for example, not accepting new patients or if the wrong PCP ID is entered), a PCP may be automatically assigned to you. You may contact Blue Shield directly to select or change your PCP after your enrollment is processed.

- Type your location and your doctor’s name. Alternatively, if you don’t have a PCP in mind, you can type a specialty to narrow your search. Or type “PCP” in the box that says “Doctor name, specialty, condition.”
- Click on a doctor’s name that you choose.
- Under Primary Care Physician ID click on View Details, and you will find your 12-digit Primary Care Physician ID.
Reminder: If the doctor you've selected doesn't have a PCP ID, they are not considered a primary care physician (PCP). You'll need to choose a doctor who is a designated PCP or find one in a different area who is. The NPI number is not the PCP ID.
