Blue Shield of California Access+ HMO

Under the Blue Shield of California Access+ HMO plan, you must receive all of your care from the Blue Shield Access+ HMO network of contracted medical groups and hospitals. You select a primary care physician within the Access+ HMO network.

Most services are covered at 100% after you contribute the applicable copayment. Refer to the Evidence of Coverage below for a detailed list of services, exclusions and limitations. You must live or work in one of the zip code service areas served by the Access+ HMO plan.

The program offers members access to a registered nurse anytime, day or night, seven days a week by phone or online. Experienced nurses are available to answer questions and provide information to help members choose appropriate care for their situation. Services include health information, healthcare assistance, self-care tools, and lifestyle counseling.

LifeReferrals 24/7
The program offers members personal consultations with a master’s-level professional anytime, day or night, seven days a week. With complete confidentiality, members can turn to this referral service for personal, family, and work-related concerns, including marriage, relationships, career, grief, legal, and financial issues. The program helps members better manage the stressful demands of home, health, and career.

Wellvolution
Wellvolution is a lifestyle medicine platform that addresses whole health across the care continuum. It provides a personalized, structured lifestyle change experience using highly rated apps, evidence-based therapies, and member-centered design.

Wellvolution allows members to create a personalized path to better health by answering a few questions about their health needs and goals and then selecting the best tool from the recommended options.

The platform can help with a range of health goals, including improving nutrition, getting better sleep, preventing diabetes, and managing or reversing chronic conditions.

Wellvolution offers a curated lifestyle medicine network with nearly 70 individual solutions from more than 35 provider partners.

Eligible members will be directed to www.wellvolution.com to access the platform and enroll in a program.
