Health Net CanopyCare HMO
HEALTH NET CANOPYCARE HMO PLAN WILL BE TERMINATED IN FEBRUARY 2027
Health Net notified SFHSS on September 2, 2026, that it will leave the California market at the end of February 2027. All current Health Net members should choose a new medical plan during Open Enrollment. If you don’t switch plans, you may face coverage disruptions next year. If you were planning to switch to Health Net, please reconsider to avoid disruption.
Health Net Canopy HMO is a narrow network plan that provides care through a small number of local accountable care organizations (ACOs), a network of doctors and hospitals that share responsibility for providing care to you and your covered dependents. Under the Health Net CanopyCare HMO plan, you must receive all of your care from the Health Net CanopyCare HMO network of contracted medical groups and hospitals. You select a primary care physician within the Health Net CanopyCare network.
Refer to the Evidence of Coverage for a detailed list of services, exclusions and limitations. You must live or work in one of the zip code service areas served by the Health Net CanopyCare HMO plan.
