Open Enrollment for Plan Year 2025

Welcome to the Open Enrollment period. You’re in the right place to get started!

At the San Francisco Health Service System (SFHSS), we believe the most important benefits are the ones you use and value. If you haven’t had your annual wellness exam, two (2) dental cleanings, eye exam, or other preventative care appointments, then now is the time to take a look at all of your available options.

Open Enrollment is your annual opportunity to change your health benefits and add or drop dependents for the next year. Elections made during Open Enrollment will take effect on January 1, 2026. Open Enrollment starts at 8:00 a.m. on October 1, 2025 and ends at 5:00 p.m. PST on Friday, October 24, 2025.

All paper applications and any applicable verification documents must be postmarked by October 24, 2025 to be accepted

Enroll with SF My Hub

SFHSS members can complete their Open Enrollment elections online through SF My Huban easy and secure system that walks you through each step of the process. Take note that SF My Hub works best with Firefox, Microsoft Edge, Safari, and Google Chrome. Please do not use Internet Explorer.

How to Reset Your Password (Actives)

1. Go to https://myapps.sfgov.org/ccsfportal/forgotPassword.   

2. Enter Your DSW Number or User Name.

3. You will get to password reset where you use your previous MFA info. to get back in.

4. For additional help, contact the City's 24/7 Dept. of Technology's Help Desk at (628) 652-5000.

We strongly encourage you to enroll online. If online enrollment is not an option, you may:

  • Fax your Open Enrollment Application Form and supporting documents to (628) 652-4701
  • Deliver your application and supporting documents in person to our Drop Box hours are from 8 a.m. to 5 p.m. at 1145 Market Street, 3rd Floor, San Francisco, CA 94103
