Open Enrollment Guide 2026

Welcome San Francisco Unified School District (SFUSD) employees to your Open Enrollment elections guide. The San Francisco Health Service System (SFHSS) administers your Medical and Vision benefits. Please note that SFHSS does NOT administer your Dental, Flexible Spending Accounts (FSA), or Voluntary Benefits. You must reach out to the SFUSD Benefits Office or follow their instructions to make changes to those benefits during Open Enrollment.

Open Enrollment begins on October 1 and ends on October 24, 2025 at 5 p.m. PST.

Executive Director’s OE Message

Your 2026 San Francisco Health Service System (SFHSS) benefits are here!

Each year, Open Enrollment provides you with the opportunity to review your benefits and make any updates that best support your health and well-being. For Plan Year 2026, the Open Enrollment period will be held from October 1 to October 24, 2025.

During this period, you may:

  • Review your current medical, dental, vision, and other benefit elections.
  • Compare plan options and make changes to your coverage.
  • Update your beneficiaries.
  • Enroll in or waive coverage.

I encourage you to take advantage of this important opportunity to ensure that your benefits align with your needs and those of your family.

In good health,

Rey Guillen
SFHSS Executive Director

Step 1 - Need to Make Updates?

Do you have changes you want to make in your benefits elections?

If YES, go to Steps 2 through 5 on how to make changes.

If NO, and you have no changes to your benefit elections, then you have no further actions to take.

Step 2 - Add or Drop Dependents

Review the dependent(s) listed in your enclosed Open Enrollment letter and dependent eligibility rules here.

Do you need to add or drop a dependent?

If NO, and you have no changes to your benefit elections, then you have no further actions to take.

If YES, complete the Add Individual Dependent/Beneficiary Information in SF My Hub to add dependents or modify existing dependents.

Save and continue through all the screens and confirm at the end to submit your changes.

Submit copies of supporting documents. New dependents must have supporting documentation submitted with their elections in order to be enrolled (e.g. birth certificate, certified marriage certificate).

If you need more detailed instructions, go to page 9 of this document:  How to Enroll Manual: New Hire/Rehire for SF Unified School District and City College Employees

Step 3 - Making Changes to Your Health Plan Benefits

Start by reviewing the Service Areas of the medical plans available to you here. There are five (5) medical plans to choose from, including four (4) HMO plans and one (1) PPO plan. Review your options below. You can compare in-network providers by HMO plans on page 10, and a summary of benefits for all medical plans.

Select your plan and follow the instructions in SF My Hub

Step 4 - Vision Benefits

Review the Vision benefits options and rates on here. You must be enrolled in a medical plan to receive basic vision benefits.

Enrollment in the VSP Premier Plan requires that all dependents enrolled in medical coverage be enrolled in the VSP Premier Plan.

Select your plan and follow the instructions in SF My Hub

Step 5 - Complete Your Elections

Complete your elections on SF My Hub

Make sure to select Save and Continue on each screen, and click Submit at the end to finalize your enrollment. Otherwise, your elections will not be recorded. For step-by-step guidance, refer to the: How to Enroll Manual: New Hire/Rehire for SF Unified School District and City College Employees
