Open Enrollment Guide 2026

Welcome Retirees to your Open Enrollment elections guide. The San Francisco Health Service System administers your Medical, Vision, Dental, and Flexible Spending Accounts (FSA).

Open Enrollment begins on October 1 and ends on October 24, 2025 at 5 p.m. PST. All paper applications and any applicable verification documents must be postmarked by October 24, 2025 to be accepted.

Your 2026 San Francisco Health Service System (SFHSS) benefits are here!

Each year, Open Enrollment provides you with the opportunity to review your benefits and make any updates that best support your health and well-being. For Plan Year 2026, the Open Enrollment period will be held from October 1 to October 24, 2025.

During this period, you may:

  • Review your current medical, dental, vision, and other benefit elections.
  • Compare plan options and make changes to your coverage.
  • Update your beneficiaries.
  • Enroll in or waive coverage.

I encourage you to take advantage of this important opportunity to ensure that your benefits align with your needs and those of your family.

Enroll with SF My Hub

 

SFHSS members can complete their Open Enrollment elections online through SF My Hub. Review your Open Enrollment Letter to see your current medical, vision, and dental elections, then click on the icon below to access SF My Hub.

Step 1 - Need to Make Updates?

Do you have changes you want to make in your benefits elections or dependent(s)?

If YES, go to Steps 2 through 7 on how to make changes.

If NO, and you have no changes to your benefit elections, then you have no further actions to take and skip to Step 7 below.

Step 2 - Add or Drop Dependents

Review the dependent(s) listed in your enclosed Open Enrollment letter and dependent eligibility rules here.

Complete the Add Individual Dependent/Beneficiary Information in SF My Hub to add dependents or modify existing dependents.

Save and continue through all the screens and confirm at the end to submit your changes.

Submit copies of supporting documents. New dependents must have supporting documentation submitted with their elections in order to be enrolled (e.g. birth certificate, certified marriage certificate).

Step 3 - Becoming Medicare-Eligible

Are you or your dependent approaching age 65 and about to become Medicare-Eligible?

If YES, and you are not yet enrolled in Medicare, you must enroll through the Social Security Administration online at ssa.gov or by calling (800) 772-1213.

If NO, be sure to apply for Medicare at least three months before your 65th birthdate.

Proof of enrollment in Medicare Part A & B are required to maintain your SFHSS benefits. 

Submit proof of Medicare enrollment by mailing a copy of your Medicare card or letter to SFHSS.

Step 4 - Making Changes to Your Medical Plan

First, review the Service Areas of the medical plans available to you as a Non-Medicare retiree or a retiree with Medicare. Next, review the Medical Plan benefits below, as a Non-Medicare retiree or a retiree with Medicare. You can check the rates here. Then select your plan in SF My Hub.

Step 5 - Vision Benefits

Review the Vision benefits options and rates on here. You must be enrolled in a medical plan to receive basic vision benefits.

Enrollment in the VSP Premier Plan requires that all dependents enrolled in medical coverage be enrolled in the VSP Premier Plan.

Step 6 - Dental Benefits

Review your Dental benefit options and costs on page 29. Select your plan and follow the instructions in SF My Hub

Step 7 - Confirmation Letter

After the Open Enrollment period ends, you’ll receive a Health Benefits Enrollment Confirmation Statement in the mail in December. Carefully review your letter to ensure your benefit elections are correct. Changes made during Open Enrollment will take effect on January 1, 2026.
